CLOMID:

Clomid is among the most widely used drug for treating fertility issues in both the genders. The chemical name of clomid is clomiphene citrate. It is widely used for fertility concerns among the females on the contrary the food and drug association of united states also known as FDA has not approved it for treating male infertility still it is sold off label and males buy clomid to treat their fertility concerns.

MECHANISM OF ACTION:

Its mechanism of action and effects will be discussed separately for male and females.

IN FEMALES:

In females clomid is the drug of choice if the infertility is due to anovulation, for example in poly cystic ovaries syndrome. It acts as an ovary stimulant by acting on the estrogen receptors, it stimulates the production of LH (luteinizing hormone), FSH (follicular stimulating hormone) and gonadotropins. Which leads to the formation and maturation of ovarian follicle, a mature enough follicle to release a healthy and fit for conception egg, and afterwards the development and proper functioning of corpus luteum to maintain pregnancy. Available in the form of pills.

IN MALES:

Fertility in some cases of males can be increased by this drug by blocking the interaction of estrogen hormone with the pituitary gland, a small gland present in the brain responsible for regulating the reproductive cycle through hormonal balance, as the pathway is blocked hence other hormones including testosterone (a primarily male sex hormone) their surge in the blood increases, improving the sperm production, sperm count and decrease in the number of abnormal sperms. Clomid UK is sold as an off- label drug and purchased over the counter to treat their fertility problems.

ADVERSE EFFECTS Of Clomid:

The adverse effects caused by consumption of this drug are listed below:

  • Alterations in mood (mood swings)
  • Gastro-intestinal disturbances
  • Nausea
  • Visual impairments
  • Hot flushes
  • Headaches

FEMARA:

Femara is the brand name of chemical substance known as letrozole. It is non-steroidal and used widely in females with anovulatory cycles and with uterine or breast cancer patients

MECHANISM OF ACTION:

IN FEMALES:

It is the non-steroidal aromatase inhibitor, as the name suggests it blocks the aromatase enzyme that is responsible for the conversion of androgens to estrogen, hence decrease in estrogen helps in ovulation, hence indicated for females with anovulatory cycles. Also used in females with tumors as estrogen level decreases so estrogen dependent malignancy size reduces.

IN MALES:

Letrozole has also appeared as a very promising drug to treat male infertilities, it is very effective in obese men where large fat deposits have too much estrogen in their system and such men suffer from obesity linked hypogonadotropic hypogonadism. Testosterone levels are low and even sperm count is deficient. Letrozole inhibits the estrogen production thus blocking the aromatization by binding the active site of aromatase enzyme, androgen level increases and fertility also increases

ADVERSE EFFECTS:

Some of adverse effects are:

  • Low mood and depression
  • High blood pressure
  • High cholesterol
  • Sleep difficulty
  • Aching muscles and joints

COMPARISON OF CLOMID AND FEMARA:

Comparison of both the drugs is as follows in the females:

No Features Clomid Femara
1. Ovulation induction Higher Slightly less
2. Live births Less chances Higher chances
3. Intra-uterine-insemination Not used Highly recommended
4. Multiple pregnancies Chances are more Less occurrence
5. Safe to use Less safe Higher safety index
6. Multiple births yes No
7. Endometrial development absent present
8. Widely used Slightly less common Most widely used
9. Still births Incidence are higher Less chances

CONCLUSION:

After looking at the above table one can conclude that femara (letrozole) have an upper hand as it is safer to use in terms of adverse effects and is associated with higher live child births. On the contrary clomid is also very effective in addressing female infertility issues but now femara has out-numbered it. In males clomid is banned but sold illegally for fertility issues however femara ensures a promising future to address male fertility concerns.
